چکیده

Results We analyzed 20 manuscripts (13 Randomized clinical trial and 7 Meta-analysis) that were published from 2005 to 2014 and that analyze 3025 neonates. Analysis of the best evidences revealed that probiotics may have beneficial effects in the prevention of NEC, however current studies have failed to control for numerous confounding variables such as breast feeding rates, antibiotic exposure, feeding practices, and environmental crosscontamination. The incidence of NEC (stage > 2) was significantly lowered only in infants weighing 1001 to 1500 g. Thus, there is not enough evidence to support the efficacy of probiotics in extremely low birth-weight infants, and future well-designed studies are needed. Trials performed at institutions with high NEC rates have observed significant benefit from probiotics, while those institutions with low NEC rates have shown limited effects. Multistrain probiotics may be more effective than single-strain products. Currently, data from about 3000 neonates indicates that significant adverse effects of probiotics are rare.

Effectiveness of probiotics in the prophylaxis of necrotizing enterocolitis in preterm neonates: a systematic review and meta-analysis.

OBJECTIVE To elucidate the benefits of using probiotics in the prevention of necrotizing enterocolitis (NEC) and its complications in preterm newborns. METHOD This was a systematic review of randomized controlled trials, which included studies retrieved from three databases (MEDLINE, Embase, and LILACS), using a combination of the terms (necrotizing enterocolitis) AND (probiotics). RESULTS ...

Prevention of necrotizing enterocolitis with probiotics: a systematic review and meta-analysis

CONTEXT Necrotizing enterocolitis (NEC) is the most frequent gastrointestinal emergency in neonates. The microbiome of the preterm gut may regulate the integrity of the intestinal mucosa. Probiotics may positively contribute to mucosal integrity, potentially reducing the risk of NEC in neonates. OBJECTIVE To perform an updated systematic review and meta-analysis on the efficacy and safety of ...

Probiotics and necrotizing enterocolitis.

One of the most controversial areas in neonatology is whether probiotics should be provided routinely to preterm infants to prevent necrotizing enterocolitis (NEC). This review provides the reader with a brief overview of NEC and current concepts of its pathophysiology, discusses the microbial ecology of the intestine in preterm infants and factors that may lead to a "dysbiosis", summarizes stu...

Probiotics for prevention of necrotizing enterocolitis in preterm infants: systematic review and meta-analysis

Necrotizing enterocolitis (NEC) affects predominantly preterm infants, who have specific risk factors leading to intestinal dysbiosis. Manipulations of gut microbiota through probiotics have the potential to prevent NEC.The aim of this systematic review and meta-analysis was to evaluate the effect of probiotics for NEC prevention in preterm infants, with a focus on specific strains, microbiolog...

The Role of Immunonutrients in the Prevention of Necrotizing Enterocolitis in Preterm Very Low Birth Weight Infants

Necrotizing enterocolitis (NEC) is a critical intestinal emergency condition, which mainly occurs in preterm very low birth weight (PVLBW) infants. Despite remarkable advances in the care of PVLBW infants, with considerable improvement of the survival rate in recent decades, the incidence of NEC and NEC-related mortality have not declined accordingly. The fast progression from nonspecific signs...
